Waiver and Release of Liability

I, the undersigned parent/guardian, understand that participation in basketball training, activities, and other physical programs at Kerley's Courtyard involves inherent risks, including but not limited to physical injury, accidents, or other unforeseen incidents. I acknowledge these risks and assume full responsibility for my child's participation in all activities within the facility.

I hereby release, waive, and discharge Kerley's Courtyard, its owners, staff, agents, and representatives from any and all liability, claims, demands, or causes of action arising out of or related to any injury, loss, or damage that may occur while participating in activities or using the facility. This includes but is not limited to claims of negligence or fault by Kerley's Courtyard or its staff.

I agree to indemnify and hold harmless Kerley's Courtyard, its owners, staff, agents, and representatives against any and all claims, damages, or costs, including legal fees, resulting from or related to my child's participation in activities or use of the facility.

In the event of an emergency, I authorize Kerley's Courtyard staff to secure necessary medical treatment for my child. I understand that I am financially responsible for any medical expenses incurred as a result of such treatment.

I certify that all information provided about my child, including health and emergency contact details, is accurate and true to the best of my knowledge. I agree to update Kerley's Courtyard immediately should any changes occur.

I understand that Kerley's Courtyard may take photographs or videos during activities for promotional purposes. I grant permission for my child's image to be used in such materials. If you do not consent, please notify us in writing.
